Lord of Life Adult Day Health Center maintains a Title VI Complaint Procedure to ensure that no person in the State of New York is excluded from participation in, denied the benefits of, or be otherwise subjected to discrimination under any program administered by the Department. In accordance with this procedure, complaints of discrimination must be directed in writing, signed by the person(s) or their representative and must include the complainant(s) name, address and telephone to:

Allegations of discrimination received by mail, fax or email will be acknowledged. All complaints processed by Lord of Life are conducted within the time frames outlined in 23CFR 200.9(b)(3). A complaint must be filed no later than 180 days after the date of the alleged discrimination, unless the filing time is extended by the Federal Division of Region Office.
