Diane is a Certified Social Worker & Certified Case Manager, working 30 years in Geriatrics, and in healthcare. Diane is also a lifelong active community volunteer with her family. Diane & her 3 siblings were raised with a strong commitment to church, faith, religion, and serving others. Diane’s extensive involvement with her church started when she was a little kid volunteering and helping with church Services, the Nursery, Sunday School, Events and Fundraisers with her parents & siblings. Diane, her Husband, and 4 beautiful children have been lifelong active community volunteers at Church, Roswell Park, Kaleida Health, Catholic Health, Hospice, Williamsville Schools, Sports, Scouts, the Marine Corps, & in the community.
Her career includes Director of Social Work and Admissions in Skilled Nursing Facilities, Rehabs, & Adult Day Program in addition to being a Social Worker & Discharge Planner. Diane's work also includes being Director of Community Relations, Sales & Marketing in Assisted Living. Her experience also includes being a Hospice Social Worker and End of Life Minister & Grief Counselor at Roswell Park Comprehensive Cancer Center. Diane has been a longtime advocate for patient end of life wishes by coordinating & serving on Health Care Proxy & Advance Directives Committees and Ethics Committees. Diane's community relations and community education work includes coordinating & presenting at Grief Support Groups and support groups of all kinds.
Diane's work at Lifetime Health embedded in 5 PCP practices in WNY and consulting for 10 Rochester PCP Practices includes being a High-Risk Care Manager and pioneer member of the Population Health Team; addressing the whole patient and eliminating gaps in care & barriers to care for Birth to Geriatric patients. This includes patient counseling and education, dementia and mental health assessments, developing patient centered care plans, providing long term planning assistance to patients & families, and assisting patients with obtaining benefits and medications. Diane's specialty is referrals and linkages to barriers to care and gaps in care. Diane has also worked & volunteered many years with Developmentally Disabled individuals and families.
Diane feels very blessed that her life & career experiences help her learn & grow spiritually. More importantly, these experiences enable her to help others and make a difference in this world with her Teammates. Diane is very honored & thankful to be chosen for this wonderful position as Director of the Lord of Life Adult Day Program. Diane's goal is to help the program & organization become even bigger and better than ever. Diane's passion is to make a difference with the Team in the lives of the individuals & families served by this wonderful program. Diane is super excited about this opportunity and looks forward to this next step in her life journey.

We believe that the staff in Lord of Life's Adult Day Health Center is our greatest asset. Our Activities Director and her staff, the personal care assistants, drivers and office staff are here for one purpose - to help your loved one thrive in a safe and nurturing environment. Most of our Adult Center staff have worked here for over 10 years. Ask them why they do this work and they will tell you it's simple - they love the people. Every day brings new and unexpected challenges and joys as we greet new participants and help them feel at home, even as we say good bye to others. It truly is a bittersweet work that we do together.
All staff are required to pass background checks and to maintain current certification in First Aid, CPR and other regulatory training requirements. In addition, the staff receives regular updates on best nutrition and personal care practices. If you have any questions about the qualifications of our staff, do not hesitate to ask. Perhaps the best way we can describe our staff is to say that any of us at Lord of Life would be confident placing our own loved one in their care!
